I need help

I explain my problem:
My son debuted with type 1 diabetes 5 years ago.It started with so little, that for 1 year there was no rapid insulin (honeymoon), and in the hospital the guidelines to follow did not indicate well, I did not know exact amounts of carbohydrates, or fast insulin unitsthat he had to put him ... in short, let's say that the rapid insulin was put "by eye", and it has not hurt us ...... 6 months ago, they changed us hospital, and of course in the firstI was consulting I did not understand anything, because my basis on the knowledge of the ratio/units was null.Educators and nutritionists explained to me and I took some other thing ... the question is that I thought they would understand what they told me but no ... my son has had many hypoglycemia with a glycosylated hemoglobin of 5.9.
2 days ago talking to an acquaintance whose daughter is also a diabetic, he explained to me above what she does ... and of course we do not match the way of applying insulin ....
My son has carbohydrate rations: Breakfast 4, Lunch 6, Snack 4 and Dinner 4.
I would like, please, that some mother or father, told me or explain their particular case and see if something of my son coincides.
I have the next review on May 12 and I would like to go more prepared to the consultation.
Thank you very much for your interest. And yes, please, someone can help me, I would appreciate it.

Napa, hemoglobin is very good, but you have to ensure that it is not for hypoglycemia, because then the quality of life is not good .., you have to point everything you eat and the insulin doses, to calculate what you need by ration. If hypoglycemia are nocturnal, decrease slow or put it in the morning ..
Give us more treatment data and controls, to see if we can help a little. But hemoglobin is very good, so it only remains to adjust a bit to avoid some hypo.

As Regina says, give us more data to help you: age, type of slow insulin, when you have dances, when you are high, if you do sports, ...
Each one has their guidelines and you have to find them ... and vary on the fly. What is good for some does not have to be for others. But with information you reach a solution little by little.

First of all, thank you for helping me and asking you for forgiveness because I have not been able to answer before ...
Tell you that my son is 10 years old, next month 11. The rapid insulin he uses is Apidra (before was Humalog, but Apidra has the option of using 0.5 units) and slow insulin is Lantus, which currently by prescription of prescription ofThe specialist puts 8 units and at night.
The nutrition specialist made us a 2,000 -calorie food plan, distributed in:
Breakfast .... 1 Lácteos ration, 4 rations in Farinaceans.
Mid -morning ... 1 dairy ration.
Lunch ... 1 ration of vegetables, 2 rations of fruits and 6 rations of pharinaceos (legumes) (meat and fish 1 1/2 rations week) and eggs 2-3 units a week.
Snack ... 1 Lácteos ration and 4 rations in Farinaceans.
Dinner ... 1 ration of vegetables, 2 rations of fruits and 4 rations of pharinaceos.
This is the distribution of carbohydrates that gave me the first time I went to the new hospital that changed me.
I initially put an insulin unit by hydrates ... sometimes it was going well and sometimes it gave the sugar downturn ... until I hearly found out that it is not so ...
So how to follow .... every 6 rations of hydrates 4 insulin units ....?
And the glucose levels that he takes before meals ... is not the same as he has 85 to have 210 .....
My friend who told you earlier, told me that her scale was 80 to 180 ... if the levels are inside those, because puts the insulin that told her ... if she goes up to 200 puts 0.5 unitsfurther...
And if you eat something extraordinary, like some cake on a birthday. If you need 6 units for the cake and have 210 sugar .... 6+0.5 ...Taking into account that in the snack they put 4 rations of hydrates in his diet ...
And at night, at dinner, I have it worse, because there are times to put the fast insulin less than it corresponds to him, they give down at 2 hours of eating, and others that I have put what he needed, and arrivesThe next day with more than 200 .... Anyway, I am a bit laded and I have been taking continuous sugar tests (every 2 hours), to get a little oriente ..... Thank you very much for your help.

You have to look at how glucose is an insulin unit, and so you can correct it when it is high.
An insulin unit usually goes down 50 glucose units, but in children insulin makes more effect.
Look to see if it is low at 3 in the morning, because if you have nocturnal hypos, it may be better to put the lantus at another time.

To my equal 1 unu of Humalog/Humaject about 50 mg lowers me, of course it is stabilized, the problem is when you upload you are not clear because it is going up and when it will stop up.

Hello napa !!My son uses the same insulin as yours, with the difference that until recently he uses it.Before I used Novorapid and Insulatard, but because I was on a honeymoon, and as soon as the endocrine is over, no insulin change. The endocrine calculates the ratio to see how much an insulin unit lowers and I talk to you a little memory but it seems to me that it was 1/75 (1 Unit of Apidra Low 75 of glucose).Sometimes it is with high values for no reason, I suppose it is due to growth since it is giving stretches (he is 12 years old) and maybe the hormonal sway. When you have days without school, p.EJ Christmas, it has higher values, the activity they develop in the school and of course sport (makes basketball 3 days/week more split) and is a regulator of impressive glucose levels. If I can help you in something, it has it.
All the best

Thank you very much for your help ... I really thank you ...
Hello Enma,, I look very identified with you ... my son the endocrine did not calculate the ratio, I did not know anything, and in the hospital change they assumed that it was already done ... Anyway .....
I have tried on my own to calculate the ratio and I get 96, so you just told me, my son needs 1 apidra unit to lower 96 glucose ....
Regarding glucose changes, and especially the holiday, it is true, without apparent reasons it has levels that you do not expect ..
My doubts, above all, is during dinner .... I do not finish adjusting the dose, I apply it just like at the other meals of the day, but it usually surprises me both up and down .... the lantus noI change it, because I don't think it's because of slow insulin (8 units).I do controls at 2 in the morning, and the same is 43 (I usually give 1/2 sugary yogurt), which 270 ....
Enma, at night is 3 Apidra ... for how many rations of hydrates?
Thank you, really, for your interest ... greetings.

Hello napa !!I talk to you a little memory, but at night I think there are 5 rations of hydrates. What happens to you, tonight, was under almost all night and I don't know if it was for the most intense exercise of basketball training, and the lantus is not going to go down one day and the other upload ...It is a bit roll, it may have to go to give it a right one, but sometimes we gave the right and then the levels were in the clouds.Iago has the freestyle and we see very well the nightlife patterns, the truth is that we are doing very well and always in case of doubt we make hair glycemia. For us it is a tranquility and for him and he takes it very comfortable, he says that it is better to prick the fingers so much.
Tomorrow we have to go to review with the endocrine and see what he tells us and if there is revision of guidelines.I'll tell you ...
All the best

Before lunch his glucose level was 119 ...
Thanks in advance for your collaboration.

I have noticed that when as more protein or fat after a few hours I get glucose even if the post has been good. That is why I usually eat meat without much fat and do not spend quantity.

You should not put more insulin before eating to compensate for proteins/fats because they are absorbed slower and the same makes insulin effect before the food and gives it low.It is best to be pending and correct after a few hours if it is high.Does your child have a bomb or bolis?I know that in the pumps there is the square bolus option with which you can put some insulin at the beginning of the food and the rest after a while. This would be the best for copious meals with more proteins and/or fats than usual.

What is what is going on, that the absorption of the HC is delayed, happens to me, if I put it before eating sometimes I have not finished eating and it gives me a hiccup because the insulin has entered me before that the HC and then mwIt is short of the rapid because I no longer have insulin or little action and I am still metabolizing the HC, so when they are meals of that guy I wear it just after eating that I have proven what is best for me.

Thank you very much for your answers ....
I am going to ask you another question ... when you make a meal (with insulin put before eating ...) and at 2 hours you check that the levels are within normal ... but at the hour or a halfDo you see that it has climbed a lot ... What is it?And when is it the opposite? When within 2 hours it is fine, and at the time or an hour and a half it has it low (hypoglycemia even) ... I am very interested in your opinion .... Thank you very much in advance ..
P.S.My son uses Boli, exactly Apidra.

Unfortunately, this disease is not an exact science, many variables are involved in glucose control (stress, physical activity, hormones, etc ...).I would soon tell you that if 2 hours after eating it is fine and an hour later this is high it could be that he fouls Lantus. If it is the revenge, that it is high after eating and then later it may be that it over lantus.

The best thing to find the basal insulin guideline is that you try to eat simple things (vegetables with boiled potato, pasta or rice without too much sauce, grilled meats ...) and you are measuring you to see how it is in the afternoon. But you also have to keep in mind that different meals have different effect on glycemia (legumes do not upload a lot and the rice hits me a climb that you see and I need more insulin fast). So you better compare the values you have after a similar meal in different days. That is, if Tuesday and Thursday eat lentils and Monday and Wednesday, eat macaroni, compare the values after eating Tuesday with Thursday and Monday with Wednesday.I explain myself?
I know it is a bit mess, but until one finds the guideline that best is a little proof and error. In addition, your child is growing and even if you calculate carbohydrates and do everything well from time to time their hormones can play a bad pass.

Napa, if you have high values before dinner, surely the Lantus does not cover you 24 h.What time do you put it on?
