The Department of Sanitat Universal I Public Salut has recently appointed Dr. Francisco Pomares, an endocrinologist at the Sant Joan D'Alacant University Hospital, a new coordinator of the Comprehensive Assistance Plan for the Diabetic Patient.
In this new stage an updated diabetes plan is being prepared, a document that will act as a guide, along with which a series of strategies will be adopted to implement improvement measures in the approach of diabetes in the Valencian Community.
And it is that 13.8% of the community of the Community has diabetes and only 8% is diagnosed."There is a significant percentage of patients who do not know and, in addition, the appearance of diabetes is being advanced 20 years as a result of obesity. Before this panorama, our goal is to achieve positive changes in people with diabetes, improving the results inHealth, "said Dr. Pomares.
Thus, different lines of work and strategies focused on the promotion of health, prevention and early detection of diabetes will be developed.The objective is to make a screening of the people who are at risk and avoid or delay the appearance of the disease."This happens to have the collaboration of professional associations, patient associations, scientific or pharmaceutical societies. Social agents are fundamental to launching these messages and motivating people with risk factors to become annual controls at their primary care center"explained the diabetes plan coordinator.
One of the early detection strategies that is intended to be extended to the Health Departments of the Community starts from an automated laboratory experience that is already implemented in the Sant Joan Hospital.This pursues the detection of hidden cases of diabetes, as well as the identification of those patients who are already diagnosed and have abandoned the monitoring program, so that they can be better controlled.In the Basic Laboratory Health Study of Sant Joan, glucose is requested to all patients and, in those cases in which it is altered, the clinical analysis system automatically detects it and another confirmation study of diabetes is carried out (theglycosylated hemoglobin exam).In addition, the detection of each diabetes case is informed of the primary care team that follows the patient.
Another strategies will focus on the improvement of school health, going beyond specific interventions and creating an educational program in collaboration with the Department of Education, which allows improving the problems of children with diabetes.
Homogeneization of therapeutic education is an important line of work that is going to be carried out jointly with diabetes and community educating nurses, so that messages that are transmitted to patients with diabetes in all centers are unified.
Likewise, the improvement of communication between hospitals and primary care professionals will be enhanced, establishing clear derivation criteria of the patient with diabetes from one device to another and taking advantage of new technologies to avoid unnecessary displacements to the patient.
The impact and risk-benefit of new technological advances will also be studied and specific actions will be developed aimed at groups of patients at risk of social exclusion, in which diabetes control is more complex.
In this way, Dr. Pomares is coordinating work groups focused on prevention, diabetological education, reorganization of work, new vulnerable technologies and groups, in order to collect and export certain working methods to the different health departments,Because "the improvement of health results is also to favor equity in diabetes care throughout the Community," said Dr. Pomares.
