Next survey

Hello, I am a new type 1 diabetic and my goal is, with a collaboration that you help me to conduct a survey that I must present in a profesorant courts to carry out a research work and well that is all.
Right now I am preparing the survey and I will publish it in the next days wanting to say between today and tomorrow.

If someone can help me and tell me any questions to add in my survey more than perfect, and if when you answer the survey os enrroyais and tell me anecdotes (about the questions) I will assault you a lot.
